The Boone County Health Department has been made aware of an outbreak of COVID-19 within Signature Healthcare at Parkwood in Lebanon. As of Tuesday, 37 residents and employees are confirmed to have an active infection at COVID-19. It is with a heavy heart that we also report 4 residents have passed away. A strike team from the Indiana Department of Health has been notified and is currently working with Parkwood to provide support, testing, and mitigation efforts. The Boone County Health Department will continue to monitor the situation.
